Lost New Jersey title: $60 replacement

The titled owner can apply for a duplicate at an NJ MVC Vehicle Center. Bring a completed Universal Title Application, your registration (current or expired) or other accepted proof, photo ID for each owner, and the $60 MVC fee. Financed or leased vehicles require additional lienholder paperwork.

I request review of the described vehicle for removal and certify that the information and documents I provide are accurate to the best of my knowledge. I have identified my authority over the property and disclosed known ownership disputes, liens and holds. Identity verification and submission of this request do not establish vehicle ownership, transfer title, authorize sale or disposal, or confirm a tow. Chain Gang must review the request and applicable requirements before accepting service. I agree to continue to Didit for driver license and selfie verification. Didit processes the identity images; Chain Gang uses the verification result with this property-removal request. I understand that a verified identity does not establish authority over the vehicle.

2. Complete the SS-58 form

Use SS-58: Authority to Transfer Interest to a Towing Company, the single-page MVC form linked below. Complete the applicable property-owner or authorized-manager section, vehicle details and signature. Keep the original signed document; staff will confirm which original documents are needed.

This form is part of the full MVC process. It does not by itself authorize immediate sale or scrapping.

3. Verify identity with Didit

Didit verifies the driver's license front and back and performs the configured selfie checks. Images belong in Didit, not in the MVC PDF upload.

Didit unlocks after steps 1 and 2. First save your request, then attach your completed SS-58.

4. Chain Gang reviews the request

Staff reviews the signed MVC form, identity, property authority, vehicle details and applicable requirements before confirming whether service can proceed. A passed identity check is not a towing approval or ownership transfer.
